This notice explains how AMPLIFE LLC ("Amplife®", "we", "us", "our") handles consumer health data as defined by the Washington My Health My Data Act and Nevada's consumer health data law, and it applies in addition to our Privacy Policy. Because Amplife® serves the disabled community, some information our customers choose to share relates to health, and we hold it to a higher standard.
What Consumer Health Data We Collect
We collect the following categories of consumer health data, only as you choose to provide them or as they arise from your purchases:

Disability and accessibility preferences. Disability categories, accessibility needs, and related preferences you volunteer in your account settings, email preferences, sign-up forms, or quizzes, used to personalize your experience.

Health-related purchase information. The products, services and events you buy on Amplife® can relate to a disability or health condition (for example, adaptive equipment or mobility products), and order records therefore may reveal health-related information.

HSA/FSA and Letter of Medical Necessity information. If you pay with HSA or FSA funds, our HSA/FSA payment partner, Gale, validates eligibility signals, processes the payment, and where applicable facilitates a Letter of Medical Necessity through its licensed telehealth partners. Final eligibility is always determined by your health plan. Eligibility and transaction signals connected to that process are consumer health data.
We do not collect biometric data, and we do not collect precise geolocation. We do not use geofencing of any kind, and we will never use geofencing around health care facilities.
Sources
We collect consumer health data directly from you (your account, forms, preferences, and orders) and from our HSA/FSA payment partner when you choose HSA/FSA payment.
Why We Collect It
We collect and use consumer health data only to: provide and personalize your Amplife® experience; process your orders and HSA/FSA payments; provide customer support; comply with law; and, with your consent, send you communications tailored to the preferences you shared. We do not use consumer health data for advertising, and we do not sell it. We collect it with your consent, or as necessary to provide the products and services you request.
How We Share It
We share consumer health data only with: service providers that process it for us under contract (for example, our e-commerce platform, email provider, and payment processors), which may use it only to provide services to us; Gale, our HSA/FSA payment partner, when you choose HSA/FSA payment; and authorities where the law requires. We do not sell consumer health data, and we do not share it for advertising. A current list of the categories of third parties and service providers with which consumer health data is shared is available on request at hello@amplife.co.
Your Rights
You have the right to: know what consumer health data we collect and with whom we share it; access your consumer health data; withdraw your consent at any time; and have your consumer health data deleted. To exercise any right, email hello@amplife.co with the subject line "Health Data Request," or adjust your preferences directly in your account settings. We will verify and respond within the time the applicable law requires. If we decline a request, we will explain why, and you may appeal by replying to our response; we will respond to your appeal with the outcome and, if you remain unsatisfied, you may contact the Washington Attorney General or your state regulator.
Deletion requests apply to consumer health data throughout our systems and those of our service providers, subject to legal retention duties (for example, order and tax records we must keep).
Consent
Where consent is required, we obtain it before collecting or sharing consumer health data, separately from any other transaction, and declining consent never changes the price or availability of our products. You can withdraw consent at any time in your account settings or by emailing hello@amplife.co.
Changes to This Notice
When we make material changes, we will update the date above and notify you as described in our Privacy Policy. Prior versions are available in our version history.
